Plaque testing is almost never covered but for echo it's relatively easy to cover by simply saying you have chest pains. You might be able to get a cheaper CCTA test by paying cash and excluding insurance.
As someone in this field, I can confirm this is 100% true. Call the facility and ask for cash pricing, paid at check-in. Cash pricing is often matched with medicare reimbursement, which is at the bottom end of payments received.
