Talked to my endo today after my MRI came back negative. He recommended TRT and gave me the option of gels or injections.
I asked him about hCG to preserve my fertility and he said he would absolutely NOT prescribe that. He said that TRT might make me temporarily infertile but he added that as long as there was no further damage to the testicles that if I were to stop TRT then sperm production would begin again but it could take up to a year. He compared it to a woman on birth control hormones, same basic thing. He did say that theres no guarantee that fertility will come back but since we think my hypogonadism is due to injury from my surgeries that if Im fertile now then he would think that fertility would be restored in the future by stopping TRT. He also said that hCG could be used later to restore fertility at such time as I wanted to come off of TRT.
He called me back later after talking to one of his Urologist associates that deals with fertility in men and they told him that hCG would not be recommended in my situation. Well, at least my endo got a second opinion on his opinion which means at least hes questioning himself.
He finally recommended that if I wanted some insurance about future fertility that I should consider banking some sperm just incase I loose fertility permanently.
My impression...
I know a lot of people do straight Testosterone Replacement without hCG and are successfully treated so I dont mind this approach to start with. I may have problems getting this guy to look at E2 and manage it so I might have to go OTC and do the tests myself, at least until I get a more experienced doc involved.
Ill be banking some sperm for insurance. I dont want to find myself in a situation five years form now where I want children but cant because I wasnt smart enough to bank some sperm when I could.
I will probably try to find another doctor to get a second opinion while I start Testosterone but I doubt that I will find one that has experience with male HRT so itll probably just be a waist of time. Im gonna ask my primary care physician what they think, hes pretty progressive but might not feel he has the knowledge to manage my HRT. Ill see if hes open to being consulted by another doc (HRT specialist...Swale?)
