Dr. Google seems to think free androgen index is not an accurate measurement for bioavailable testosterone in men. The topic is still mildly confusing to me as I am still learning. Surely, a high free androgen index indicates relatively low SHBG to total testosterone ratio which should be a good estimate for amount of free testosterone not bound to proteins and available to cell receptor sites. When requesting lab tests do the experienced members of this board use specific values in isolation to guide their dosages? Or do they take a more holistic approach? I suppose the latter sounds more reasonable to myself. The crux of my confusion is why is FAI more accurate for free T in women and not men? If the endocrine society don't use FAI then why do doctors still use this test?
