I'm new to the forum but have been reading here for a while. I am currently on TRT(2 -5g packets of Androgel daily) for Secondary Hypogonadism and have been taking it for over 18 months now. I'm 58, 140lbs and workout religiously 3x per week. One problem that seems to be happening while on TRT is that I still have penile sensitivity problems and not being able to achieve an orgasm. Its really starting to affect my marriage and I want to change this as quick as I can. My wife is being very understanding but, I feel like I'm failing her. I've talked to my doctor several times about this and asked him for any prescription or protocol that could improve this situation and he told me there's nothing he can do. I've also talked to my endo and he put me on an anti-depression drug that did absolutely nothing but make my head feel like it wasn't attached to my body and threw them out after taking them for 3 weeks. So, I'm no closer to resolving this situation, than I was 18 months ago. The TRT seems to be working, my total T is back up to 536 from 236 and all my other levels are in midrange (E2, Prolactin, Free Test, etc.) except for my LH and FSH levels (under 1.0) which are low and the doctor said that is to be expected when on TRT. Does anyone have any suggestions on what I can do, drug, protocol, etc., that could help in my situation.

What lab did you use? You should get labs for the most sensitive estradiol assey available. Labcorp calls this estradiol, sensitive.
 
Thanks for the replies. I have called my doctor to get a copy of my last results so I can post them. I go to Quest labs for all my bloodwork. In regards to the E2, they call it E2 Male Extraction method which gives the Estradiol results.
 
Okay, you got the right E2 test done then. Very good. Wow, 10g of Androgel/day but your testosterone is still within range. Have you had your DHT checked? It may be grossly elevated in which case the shots might be a better option for you.
 
While gels are the best at mimicing the body's natural production they are not always best for the person. At 7.5g/day my total T actually dropped BELOW my pre-TRT levels! You need a lot of skin for that much T to be absorbed properly.

Also, DHT may not necessarily be high. Mine never even got INTO the normal range while on gels...highest it ever got was 65 on a scale of 155-550! On injectables it's at a decent range though.

I've never heard of the penis sensitivity thing though...at least not caused by the drugs. Maybe Headdoc has some insight?

I have had this problem for over 10 yrs. I could not have sex with my wife. I found out about high E2 Estradiol and when I got it down the range is 10 to 30 my orgasms came back and my ED got 80% better. If my E2 goes up past 35 I start having problems. I am now 61 take Cialis 20 mgs every 72 hrs. as per my Dr. and doing this is keeping my BP in check. So try taking some Indolplex/DIM see if this helps. You said your E2 was in the mid range this is to high.
