My Prostate Issues

Ive posted this on other forums, but perhaps Dr Scally and others on this site have some insight on this:

First and perhaps most importantly, I'm only 27 years old. My test level is about 350ish. I began losing my hair when I was only 16, and now at 27 my male pattern baldness is pretty evident. My dad has a full set of hair. My uncle on my moms side, and my grandpa on my moms side both suffer from severe male pattern baldness. There is no history of prostate cancer in my family.

Last summer (July of 2011) i started suffering from urinary issues. I felt like I had to urinate but when I went, nothing was happening (or very little). Went to the doctor, and based on symptoms alone, he said prostatitis. He gave me 1 month worth of cipro. At the end of that month, I went to another Dr who did a DRE and said normal prostate. Eventually, all was well.


3 months ago, march on 2012, I seemed to have symptoms of an std. I had a new partner, and after a couple crazy nights of sex ( 8 times in 12 hours), I began having the same urinary symptoms, but this time, with burning and a discharge. Std test after std test found nothing. Doctor prescribed 2 weeks of cipro, then another month of levaquin. It did nothing except give me serious bladder pain. I went to an std clinic and was given 7 days of doxy that got rid of the discharge and burning. Dr at the ER did a DRE and said normal prostate. He did an abdominal ultrasound and said my prostate appears a little big, but didn't say anything else.

Just this week again, I have a "full" feeling in my prostate and again a feeling like I have to urinate with small amounts. Also, when I have a bowel movement that requires me to strain, there's a small amount of semen like discharge.

What do you guys think this is? Prostatitis but no tests showing bacteria? I really want to avoid cipro and levaquin without assurance there's an infection.

Could it be BPH at my age?

Could it possibly be prostate cancer? I know it's not likely at my age but I am a bit worried. No doctor will give me a PSA because of my age.

Finally, I've been under a lot of stress in the last year or so. Nearly every ache and pain I have had begun since this stress. Could this all be in my head and be stress related? I
What can I do? My quality of life isn't the greatest with this condition. Doctors seem clueless and keep pushing cipro on me. If this is prostatitis, doesn't the constant inflammation cause concern for prostate cancer?

Is BPH even possible at my age?

Does prostatitis cause cancer?
Although prostatitis can cause you discomfort, it does not cause cancer. Some doctors use a blood test called the prostate-specific antigen (PSA) to test for prostate cancer. If you have prostatitis, your PSA level might go up. This does not mean you have cancer. Your doctor will treat your prostatitis and may check your PSA level again.

Can prostatitis be passed on during sex?
Sometimes prostatitis is caused by a sexually transmitted organism, such as chlamydia. However, most cases of prostatitis are caused by infections that are not sexually transmitted. These infections can't be passed on to sexual partners.

What if my prostatitis is not caused by infection?
Because doctors do not yet understand what causes prostatitis without infection, it can be hard to treat. Your doctor might try an antibiotic to treat a hidden infection. Other treatments are aimed at making you feel better.

Nonsteroidal anti-inflammatory medicines, such as ibuprofen (two brand names: Advil, Motrin) or naproxen (one brand name: Aleve), and hot soaking baths may help you feel better. Some men get better by taking medicines that help the way the bladder or prostate gland work.
 
Yeah, Im pretty sure its not a bacterial infection... *except* I do wonder if whatever I had that caused the discharge, which was cured by the doxy, still remains within my prostate? If that makes any sense.

Im willing to try some anti biotics, but they didnt help much before, and cipro and levaquin arent exactly the safest drugs out there


My latest E2 was well within the normal range (although it has been high in the past and dropped for reasons unknown)

I am curious tho... you often read that prostate cancer is closely related to inflammation. Isnt prostatitis, in essence, inflammation on its own?
 
They are not related very closely. Only that PC can surely cause inflammation of the prostate in cases. Men live their entire lives past 40 with BPH and never get PC, There is no direct corrolation.

From your description, it does sound like an infection was there. The "feeling of urge" and all. Keep in mind here are two test points I have identified:
1. The inability to open to flow as caused by drugs. benedryl (sleeping aids & Antihistamines) will cause this in a few days for many. Narcotics (hydrocodone) is a sure fire way to have this issue.But this is the inabilty to start as a feeling... I used to confuse. But in this scenario, its not prostate at all. Its neurological with the "plumbing/valves"....
2. And now that I am more experience in life (older - lol), urinary difficulty as related to true BPH is denoted as the feeling of "starting to flow, and quickly hitting a wall where it slows, or stops". But the valve is open, and it also feels like slight back pressure I think. The scene in Zoolander with Jerry Stiller could be an example of either IMO.

Also I am not sure the exact locations that all antibiotics act, but I would suggest attempting to get him to get you are run of Clarithromyacyn (Biaxin) for 8-10 days, as this is commonly prescribed for these areas. Clymydia is a good example of a bacteria that can go rampant and difficult to diagnose after the first few days. I may be what you had. Mayber worse thought considering the STDs on the rise. There are also those that would shutter to think you were given a quninolone (cipro) for other reasons, might even argue that it could have compounded. Who knows. I am not so sure these days if a quinolone did not end my good erections. But we are quick to point blame. I can guarenteed Avalox (spelling) ended my life long seasonal bronchitis ONCE and FOR ALL... LOL...:D

If it were me and you are still having the "feeling", I would try the Biaxin... Or at least inquire to him.. But surely if you are still having issues you have told him?

Im pretty certain there was some sort of infection at one time -- when I was having the discharge and burning feeling. Cipro did nothing to fix it. Doxy got rid of the discharge and other symptoms for a couple of months.

Just this week have I started having the "need to go" feeling all over again, but no discharge, so Im not sure if its related to the STD type event. I never did find what type of STD it was, but test after test showed it wasnt chlamydia or ghonnarea. I think our testing for those 2 STD's is pretty solid... I suspect it was one of the less talked about STD's like mycoplasma genitalium or u. urealyticum. It couldnt have been trich because I didnt take the right medication to get rid of it. Anyway, the doxy worked and the discharge and burning is completely gone. I think this may be an unrelated issue to this STD incident.

Im on day 2 of the broccoli treatment. We'll see how it does.
 
I think Doxy is considered a "deep tissue" antibiotic, and not so sure how accurately it applies - IN MY DIMENTED MIND. However, I will say that I recall a reproductive doc gave myself and my wife that as a phase one type preparation for 10 or 20 days. Seems like it was an incredibly long run at 20 days. So with that said and from a repro doc. Consider. I Still say a run of Biaxin is in order...:D
