Nipple isues - pain

I’m running 250mg Test, with 12.5mg Aromasin x5 per week.

I have been feeling great for 2 months, no problem whatsoever. I got a tiny lump of gyno from not managing my E2 properly a while back, but not feeling high E2 right now (no bloat, no acne, no high BP).

I even checked my E2 a few days ago and it came back at 27pg/ml with Immunoessay. On another dose it came back at 29 and I felt amazing, and the time I got the gyno it came back at 44 and was very bloated.

Well, for the past two days my small lump on the nipple has been killing me, it hurts like crazy but it doesn’t burn at all (like it did first time I got it). And it doesnt hurt to the touch and is not sensitive either, it’s like a deep sharp pain in the lump.

What the fuck is going on? Do I up AI, take tamox? Why tf is this happening?

HELP

Recent labs? As of this incident?

Labs for sure and get tamoxifen in hand for gyno if you're prone, which it seems so.

Also injection schedule, ester, so on will be helpful to get E under control. But again labs.

Sorry to hear this, very frustrating given the E2 level. Did you happen to get a prolactin level? I know you are on test only but would be good to know what your baseline prolactin is. Are you by chance taking any dopamine blocking medications such as antipsychotics?

I would do some research on Tamoxifen protocols. They need to be run when the gyno is fresh / new, otherwise they're not effective.

I did run a Tamoxifen blast when I first got it, like 80mg for 1-2 weeks and then 20mg for a while and nothing happened.

I didn’t check my prolactin and not taking anything else like that :(

I do take Dutasteride though

You already have existing glandular tissue from when your E2 was higher 44. Once gyno tissue exists it can flare up locally even when serum E2 looks good.

Estrogen action in breast tissue is local, not perfectly reflected by blood E2. Inflammation, receptor sensitivity, prolactin signaling, or even minor hormonal fluctuations can cause acute pain.

Seems like what you actually have going on points more toward inflammation of existing tissue, not new growth.

Tamoxifen blocks estrogen at the breast receptor level, which helps for many. Your initial approach was aggressive but short from my personal opinion. Typical approach usually starts with 20/40mg/day for 4 weeks +.

Tamoxifen prevents progression and calms activity far more reliably than it reverses established tissue. So “nothing happened” doesn’t mean tamox is useless, it means the gland likely stabilized rather than shrank.

Your Dutasteride blocks DHT hard, and shifts androgen/estrogen balance toward estrogen signaling in tissue. It removes DHT’s local anti-estrogenic effect in the breast tissue.

So even with normal serum E2, breast tissue can become more estrogen responsive when DHT is suppressed.

Just a possibility but worth looking at.
