First, go to the private labs md website and sign up for the monthly newsletter. That way you will get the coupon code emailed to you every month.
Next, you will order the tests you want either through the website or the 800 #. From there they will email you the requisition forms about 30 mins later. You can either print out the forms and bring them with to the testing center, or you can have the forms faxed over to the testing center of you can't or don't want to print them.

One other thing I will point out is that if you have the choice between using either LabCorp or Quest Diagnostics in your area, there are definite differences.
**labsmd.com uses Quest - and on the Quest Female Hormonal Panel (this is the test most of us order) they use the lc/ms-ms method for calculating testosterone levels. This matters because with this method you will be able to obtain test values OVER 1,500. The labsmd.com female hormonal panel is $72.99.
**privatelabsmd.com uses LabCorp - and privatelabsmd/LabCorp's Female Hormonal Panel uses the method that will only show >1,500 and will NOT give you your test levels if they are over 1,500. The privatelabsmd.com Female Hormonal Panel can be had for about $57 with coupon code. So you would also have to order a separate Total Testosterone test that does use the lc/ms/ms method to get that total score if it's over 1,500. I think that test, added to the female panel, would make your total over $100.
**we usually order the Female Panel as it is about $100 cheaper than the Male Panel.
**make sure you choose the "male" option when filling out the form, as it will ask, and you need the correct male reference ranges when you get your results.
