Unless you're diabetic, i'd probably start with adding on 1 at a time, checking blood work and adding another one every month or so.
Definitely interesting info on pioglitazone...and I loved finding out how the SGLT-2 inhibitors work (my cursory understanding of them used to be - well shit, that's gotta tank the kidneys sending all that glucose out into the urine).
My apologies if you don't actually listen to Attia,.in which case you have no fucking clue what I'm going on about...lol.
Totally....the recent Attia podcast was a gold mine of info for the meds and diabetics.SLGT2 ones such as Jardiance have really good secondary benefits and i am Type 2 Diabetic yes
My Doc only gives me Metformin and basically tells me to go with 4G a day of it. Only running 3G which has shown some improvement but since adding Tirza my glucose levels have been so much more stable so i added Jardiance and my fasted glucose is about 75-80 (used to be 95-100)
I want to take the next step and amplify my Insulin Sensitivity further by introducing pioglitazone at 20-30mg's.
Also in a cut at the moment and lost about 8KGs and want to make use of everything i can to do become healthier.

It's a shame the state of healthcare is "what code do I put this under to satisfy insurance"....as opposed to optimizing actual health.
I am lucky to be in a position to tinker somewhat..But there are virtually no doctors within 100 miles of me that would even consider a non-cookbook approach to medicine (obviously for fear of liability and of course the medical establishment likes to stay comfortable).

And when asking for Mounjaro/ozempic doc says no, despite bloodwork and family history showing a chronic insulin resistance
So yeah, after quite some research i wanna do the triple therapy and improve things for good

You know your doc is only doing that to save whatever medical conglomerate "pays the bills" (whether it's socialized medicine, private insurance or the like).
I have heard multiple times about doctors expressing concern over "useless" and "expensive" tests and meds...like fuck off and let me decide since ive figured out you're full of shit...(That's me thinking it to the doctor but not saying it...and leaving without getting my way...lol).

I can't fathom the logic of a medical system that just has us live diseased with no quality of life for the last 20-30 years of our life...rather than have some quality into our 90s, minimal health care expense with some preventative health measures in our early years...and then just drop dead of old age (as opposed to spending another 2 years being kept alive by machines and truly useless interventions as they seem motivated to do currently).

The good thing about the American healthcare system is that the entity paying for the first 60 years is different than the entity paying for the last 20. I'm using SAMay I ask who you got it from? Some of the CN vendors say it is bac water but we have come to find out it was just sterile saline. The vendor begged apologies as "language barrier"... that may be the issue.
Maybe. The first vial dissolved instantly, since then every single one took longer and longer. I think the tirz is fine, I definetly felt the effects and had no allergic reaction at injection site.I have no problems instantly dissolving 10mg in 1ml saline water for injections. Something may be off with your tirz.
@Ghoul, how do I smoothly transition from Tirz 7.5mg to Reta? Should I start with 2mg? Thanks.
Thanks for the response, sir - straight to the point as always. I’ll do exactly that.I wouldn't go from 7.5mg Tirz to the bottom of Reta. 4mg, one step up the titration ladder, along with being a much closer match on the receptor affinity scale, seems more reasonable,
4mg Reta isn't going to lead to crippling sides from going up in appetite suppression too quickly coming from Tirz 7.5mg. If anything, you may experience an appetite rebound, but better a little too weak than too strong,

I use a Freestyle Libre CGM for real time values. Fasting values have gone from 180+ to under 100. I have had to drop my slin dosages by 1/2 to get these values without going hypo. The daily graph has leveled out, showing short rounded increases in BG, instead of high sharp peaks.

Part is due to appetite control, and a large part due to an increase in insulin sensitivity.
And all this time I've been trying everything to control this by steadily increasing Slin. I hit a point of diminishing returns. A1c was up to 11. Have to get it down Asap for a surgery, that's when I tried Ozempic. ( Insurance wouldn't cover Tirz.)
