advice needed- -primary sufferer-still unsure- testicular problem leading to low T

im 19 and have really atrophic testicles- this has led to low testosterone. i didnt have mumps/hard kick to the balls or a genetic disorder, and one of my balls used to be really big but shrunk over the last couple of years to tiny, the same size as the right ball which was always smaller. ive seen an endo and a uro and neither could tell me whats wrong. I had a varicolele, and had it removed to see if that improved things. too early to tell as I havent noticed any regrowth or improvements.

i have the usual symptoms of low testosterone, and would like to improve body and mood. im considering going on TRT again, but i want to take HCG as last time going on TRT made me temporarily infertile and im worried that as my lh and fsh are tripple the normal values, that HCG wont be strong enough to keep my already fragile balls up and running when submitted to trt ( i produce sperm at mo but dont want to lose it- feels like walking on egg shells.

Because im worried about further atrophy and desentivity to HCG ( worried about the pituuitary becoming immune to HCG or it not being strong enough) ive been looking at other ideas. I was wondering if even if one has really low T, will lowering estrogen further improve matters, or does one need a decent base line of T regardless.

I also had a varicocele embolization (vein in scrotum removed) to see if that will improve my testciular health (as they used to be healthy) so of course i dont want to undermine any potential growth by taking TRT.

Any thoughts about my predicament would be really appreciated. as per usual my results are below. Id rather just try and lower the estrogen with drugs, but it may be that my estogen is already pretty low, or that my T just isn't enough.

if anyone could take a look and have a think about what I should do, or could just give their experiences of these approaches id be really greatful. Test results below

Are you in the UK if so I feel sorry for you getting help is dam hard. Your levels are dam low but your LH and FSH are like mine were when I first got tested. HCG will lower your LH and FSH but make your Testis and other glainds make some T how much is different for every one.

This was suggested to me at some point...

If youre worried about your long term fertility, and assuming you are fertile now, you may want to bank some sperm before starting TRT again. As it is you may have reduced quality of sperm so it may be important to save some especially if youre condition may degenerate your testicles further. Its well worth the cost to have a little insurance for a future family. Of course having a successful pregnancy even with banked sperm is not guaranteed but having no sperm is a guaranteed to keep you from having children of your own. Adoption is always a possibility though. Also, with hCG perhaps they can induce sperm production for a week or so so you can bank some.

hey im planning on banking sperm. i was put on trt before i was told of the risks, so ive been of it for like 5 months to see if my fertility has returned. I am hoping so. after that im either going to go down the anti estogen root, or use trt and hcg, which ever one is more likely to maintain my testicular health.

any other thoughts guys.
 
From my understanding, hCG in primary sufferers can spur whatever testicular function is left so its not a total waist unless youve lost ALL testicular function. And in that case youd be infertile anyhow. I think trying hCG, or any other drug that promotes spermatogenesis, if you want to bank sperm is probably worth it especially if youre not fertile after a while off of TRT. From what I hear it can take up to a year to become fertile again after stopping TRT. Plus, I think I heard that hCG has other non-testicular benefits to emotion and mood so its not a total waist even in full blown primary hypogonadism.

Im just going to be starting TRT and I dont want to chance permanent infertility so Ive banked some sperm just for insurance although I understand that its probably unlikely that I will have permanent infertility. I just dont want to regret that I didnt bank some later if infertility persists.
