I don't think it's the crystals: you can see crystals in the vial → seems too big to get both into and out of a needle/syringe (or the shear stress & heat of withdrawing into the barrel them would be enough to whack back into solution [do you ever see crystals in the syringe?])
But if you actually injected intact crystals, they're long gone, probably seconds after injecting them (heat and constant movement, same thing that works for fixing crashed vials)
That doesn't mean it couldn't cause PIP, just not a likely candidate
