DFHT Questions

I have some DFHT questions that I hope someone can answer:

How much rest between sets is generally recommended?

What kind of weight increases are expected? Is the loading phase similar to HST where you start sub-maximally? In other words, in the 3 weeks of loading, should I go for 95% of 5x5 max, then 100%, then in the last week go for 105%? Or do I try to lift my max each time? What about the higher rep sets? What do I do if I can't progress or fail repeatedly? Is that a sign of overtraining? How often typically would you expect to increase weights?

What about rep tempo? Are we talking explosive positive and 2-3 second negative or what?

What about warm-up sets? Should these be done for every exercise? Just once at the start or what?

What about when the 5x5 sets turn to 2x12 for the week... what typical percentage of your 5x5 weight should you shoot for in the 2x12?
