Help with latest Labs

Background: I am 31, I have been on 10gms of AndroGel for about 8 months now. I was started at 5gms and bumped up after initial labs.

Labs from 4/05/05 - Blood Drawn at 10:50 approximately 4 hours after application.

Total T - 570 N 241-827 NG/DL
SHBG - 12 N 7-50 G/DL
Free T - 200 H 34-194 PG/ML
Bio T - 428 H 84-402 NG/DL
LH - 0.4 L 1.5-9.3 MIU/ML
DHT - not checked, but really needs to be. Doc just never does and my 1st Endo didn't seem to care what it was. <-- Searching for new Endo.
E2 - not checked, but was normal when using a higher dose, so I no reason to think that at a lower dose it would be out of range.

CMP all normal
Lipid panel all normal
Red Blood Cell Count 5.71 N 4.20-5.80 mill/MCL
Hemoglobin 17.3 H 13.2-17.1 G/DL
Hematocrit 50.9 H 38.5-50.0%
All other values for CBC normal
Platelet count in the low normal range though 185 140-400 THOU/MCL

My question boils down to, how can my Total T be in the middle of the range and my Free and BIO T be out of range. My H&H only concern me b/c I donated a pint/unit/bag full of blood just 2 weeks before this blood work was done, so I was expecting that to come back in the normal range.

Any comments, or suggestions would be appreciated. I would like to add HCG and have been trying to convince my Doc, based on previous Total T tests, I had some room to spare, now I don't see it happening, meanwhile my boys are going away quickly.

IMHO Since your SHBG level is low your T is not being bound up. Which allows your free T levels to remain high.
 
Yes, I believe Buck is right. It's not a bad thing. I have basically the same thing; after my 5-week labs (100mg cyp/wk) , my total T was good but not in the upper quartile, but my free T was at the high end of the range. My SHBG was at the low end. Doc kept me at that dosage and added hcg.

Yes, on 10mg of Androgel qd, you definately need to get your DHT checked. There isn't a free or bioavailable DHT test, but we can infer those values based off of your total DHT (e.g. if your DHT is high-normal, then your free-DHT is probably above-normal).
