LOW thyroid panel numbers

I recently got some bloodwork done and realized my thyroid numbers were out of range, I've never took tyroid meds, I only currently Blast and cruise so considering these numbers what do you guys recommend, I'm all for self prescribing replacement t3/t4 if need be.

If you had any hypothyroid symptoms, ie low energy, poor cold tolerance, with those numbers they'd consider it primary hypothyroidism and start you on 50mcg Thyroxine (t4)/day,

Take it for a month, run numbers again. If you feel good, and the Free t4 isn't above normal, and TSH isn't lower than .35 you're good.

Ideal free t4 is top 1/3 of range as long as tsh isn't below .35.

Be patient. Can take weeks to feel effects, and they can continue to improve over 6 months. Very important to take tab 30 mins before food. Very first thing in AM. Avoid coffee for first hour. I keep bottle bedside and grab it the moment I wake.

Optimizing T4 made a *huge* difference to mood, energy, fat started melting away, general well being much better. I felt it effects almost right away, but benefits continued to increase over the first two months.

120 pharma tabs 50mcg is like $3 from India PCT places. I'd avoid ugl for this.

Recommend Lethyrox brand from Intas.

So cheap might want to get a few 25mcg bottles so you can easily increase or lower the dose,

If it works for you, it's a very "wtf why didn't I do this sooner" kind of thing. A profound impact.

How can he be primary hyphothyroid if is TSH is below 2.0

It's should be skyrocketing high and with low T3/T4 if he has primary hyphothyroidism
 
Primary Hypothyroidism is characterized by an elevated TSH and low levels of FT4 (and often FT3) due to a failure of the thyroid gland itself to produce enough hormones

A normal TSH with low FT4 and FT3 suggests a different issue, often pointing towards central hypothyroidism, which results from problems in the pituitary or hypothalamus rather than the thyroid gland itself.

In summary, a normal TSH with low FT3 and FT4 typically indicates central hypothyroidism or another underlying condition rather than primary hypothyroidism.

I could've been clearer. With TSH in normal range, Free T4 low, and SYMPTOMS (which I noted), It would be treated as primary hypothyroidism would. The diagnosis would be subclinical, but in terms of treatment, it's a distinction without a difference.

Though imo, unless they suddenly developed or are severe, it's hard to know
you're even having symptoms in that state until they're alleviated.

Yes the treatment is similar BUT he could have an.underlying cause at the moment that maybe is not related specifically to it's thyroid so it's worth investigating.

I figure if someone brings it up here, they've probobly decided to treat themselves vs going to the doc (which is clearly the best choice). Even mentioning T3 made me want to point out that most likely, he'd end up on low dose T4, no T3, after the usual routine scans to eliminate any serious causes,

Appreicte the amazing info brother! I do have pharma t4 and pharma grade t3 on me right now. Thyroid has always been in the back of my mind as a potential issue as I do have lower than normal energy levels despite my hormones being optimized whether I'm blasting or cursing. With that being said, I will begin 50mcg t4 first thing tomorrow and continue down the line until bloodwork in a month. Thanks again!

There is little to no reason to treat this yourself. Go for a proper evaluation to a doctor. I would investigate the cause of your Secondary hypothyroidism, as investigating the pathogenesis of it might shine a light on some other important underlying pathology.

Right ... I would definitely retest once you're off cycle. Also, it's going to be hard to get a proper medical evaluation with you being on all these drugs, so if you plan on blasting for the foreseeable future, taking T4 might be your best bet. I do not recommend this as the best course of action, but realistically, what are the chances of you dropping to trt levels and getting a nother set of labs, am I right ...?

When I drop back down to trt I will retest and evaluate. In the past I’ve only gotten a tsh and it was slightly higher than what my bloodwork shows now so still in range, as for the free t3 and t4 levels I don’t have any prior bloodwork to show for. Again I will continue to blast and cruise for as long as need be, with that being said I will now implement in t4, yes I should probably be a little more educated and have my doctor evaluate but this is the same doctor who wouldn’t up my trt dose of test above 75mg a week so that says a lot. So theoretically what would you advice my t4 replacement dose be?
 
Well I go to the doctor for low thyroid and they only look at TSH. Your TSH says your thyroid is performing exactly as it should. A slow thyroid would have a TSH of 4.5 or greater. A fast thyroid is between one and two. Your not hypothyroidism at all. If your TSH was less than one you'd be hyperthyroid.

yes that's only test most primary doctors take and look at which obviously doesn't look at the bigger picture, for example and seen above in my results, "free t3 and free t4". Which show less than the bottom of the reference range. So yes my TSH shows in range where as again free t3/t4 are tanked. My guess is most doctors only test TSH so they don't have to deal with another possible issue of any sort of hypothyroidism so aren't testing all the thyroid panels. I could be wrong just my guess.

It is possible to have hypothyroidism with normal TSH.

Those 3 that weren’t shown weren’t tested, they were just being shown for future reference in case I need/want to get them done.
Yes, I comepelty agree and plan on testing again when returning to TRT to get a better look at the big picture.
