New 2025 case report – Mucosal malignant melanoma after Melanotan II nasal spray use

Case details:

• 22-year-old healthy white female, Fitzpatrick skin type II

• Used Melanotan II nasal spray for tanning purposes

• Noted oral pigmentation shortly after starting the spray

• Presented with a rapidly growing pedunculated nodule on the attached gingiva over the upper central incisors

• Histopathology confirmed mucosal malignant melanoma

• Treatment: surgical resection followed by immunotherapy

Authors conclusion:

“Risk factor analysis suggests that the most likely cause of the mucosal melanoma in this healthy young patient is related to the use of Melanotan II nasal spray.”

Weird. Thanks for sharing. The same mechanism of action possibly related to melanoma for melanotan 2 and melanotan 1 is the melanocortin 1 receptor, and MT1 has been proven to reduce the risk of melanoma. I don't see how MT2 could be any different. Possibly higher doses of MT2 could cause a more rapid effect, which when combined with a strong acute stimulus like high UV could cause some sort of shockingly proliferative environment in melanocytes that leads to cancer. That's why I've written off any of the previous cutaneous melanoma studies as tanning beds are an obvious confounding variable, but the fact that this was in her mouth is concerning. I read that renal infarction case study. To my knowledge, this wasn't cancer, it was an acute loss of blood flow to the kidney. The guy took 10 mg per injection twice within a period of 6 months and 7 mg 3 weeks before the admission to the hospital. I only mention it because that's a pretty fucking wild dose, and melanocortin agonism is known to activate the sympathetic system, like any stimulant, which can raise HR, raise BP, and potentially cause the blood flow to the kidney to be constricted leading to renal infarction. I don't exactly buy this given it was 3 weeks after his last shot that he had acute kidney damage, and the researchers just claim that they ruled out all the other common causes and essentially MT2 was the only weird outlier. They also mention he was drinking, and maybe the fucking batshit doses of a MT2 caused consistent overstimulation and vasoconstriction which damages the blood vessels connected to the kidney and when the guy went on holiday the alcohol tipped him over the edge. His BP was 165/95 mmHg, so seems possible. In my opinion, I wouldn't be surprised if there's a case study on someone taking 10 grams of caffeine having kidney damage. Still going to take my 400mg personally pre-workout. Devils in the dose. Maybe be smart if you're on MT2 to get your kidney blood test regularly.
