PSA levels - when is it a concern?

Looking like I need to go to optimal test doses and focus on a health phase.

Been blast and cruise for about 3 years.

Just finished a contest prep with several DHTs .

I’m nearly 40, psa been creeping up the last 3 years with PED use … .8 ,1.2, 2, 3, now 6.6!

I had an MRI scan just this past fall to check out my prostate and was told not enlarged or no masses. So I’m mostly hopeful it’s just from using DHTs too long.

Any thoughts? How long does it take to drop if peds are stopped? Plan to stay on TRT.

Thanks

Sometimes there can be a big unexplained spike in PSA. Stool got backed up, irritated the area but in the end the trend is most important. Your trend is bad and should not be ignored! Does it matter how long you need to come off for? Go back to true TRT and retest every 3 months

Thanks guys.

I’ll see my primary soon and go to true TRT and retest when the doc says.

I don’t have urinary trouble and the mri was ok just months ago.

Still nervous but hopeful it’s just from blasting mast.

From grok:

At age 38, total PSA 6.6 ng/mL with free PSA 0.58 ng/mL yields percent free PSA ≈8.8% (calculated as free/total × 100).

This low ratio typically signals higher prostate cancer risk (>25% biopsy-positive odds in 4-10 ng/mL range).

However, your PED use (testosterone + DHT derivatives) likely drives elevated total PSA and disproportionately lowers % free PSA without cancer—clear October MRI strongly reassuring.

Cancer risk very low; retest PSA/% free off PEDs (expect normalization in months). Consult urologist.

I have had prostate cancer now twice. First time was at 47 they sent me to the urologist when my PSA hit 3.9 ... Its something you want to get in front of because so much easier to deal with while its in the prostate bed. There is a new urine test that can really help predict if you need to look at a biopsy or something more .... MyProstateScore version 2.0 ... https://www.auajournals.org/doi/10.1097/JU.0000000000004421 Best of luck to you.

Did they have to castrate you hormone wise?

Not the first time but yeah the second time they did. First time was just surgery second time was radiation and castration. Second time was a lot worst. First time you heal from surgery and you are going again second time you get poison injected into you for 2 years and feel like shit during the whole time. Knock on wood though no cancer detected now for 18months. Stay healthy everyone and have a good Christmas.

guys I made a mistake noting I had an MRI.

What had was CT without IV contrast. What I’m reading says need MRI specifically.

The CT only shows normal prostate size, not as useful for prostate cancer.

I’ll be in touch with my primary soon and start running this down.
