Dear Dr Swale or anyone else who would care to advise,

I was wondering if you could advise me on the most appropriate form of treatment for me. (lab results at bottom of post)
I am a 19 year old male, 145 pounds with severe primary hypergonadism.
I have the usual symptoms of fatty chest and sknny arms and i am also mildely depressed. I previously went on TRT but it caused my lH and fsh to disappear, (from triple the normal level) after only two months and my sperm count disapeared. This is very depressing for a 19 year old as you can imagine, but more importantly i am unwilling to subject my body and balls to these conditions for the rest of my life. I therefore have been looking for an alternative to just taking TRT (as administered on its own in the UK where I live). the endochronologists in the UK i have encountered are aweful by the way when it comes to this kind of thing!!

i basically want to maintain what testicular function i have, with no loss of sperm or testosterone at all and would like to improve my body and mood ( I am not interested in huge muscles) I also recently had a varicocele embolization to see if that will improve my testicular health- the procedure was a month ago and so far i feel no change in the balls but im holding out hope.

My options are as follows. One, try and lower my estrogen through DIm (at the moment i just use zinc) enough to see an improvement in my muscles as the testosterone i produce myself becomes more effective as it doesnt have to battle estrogen.

The second is to use TRT with HCG. my only concern is that HCG is hard to get in the Uk, and that I am worried sperm production and the delicate state of my fertility/testicular health will be effected by the TRT and that the HCG isnt effective enough.

The third is trying to raise free testosterone, but i have heard this is hard to do, and lowers DHT and indeed can cause atrophy by effecting the pituaitary feedback loop.

As i have said i dont want to get bulky, but to be lean would be great. Most important is to preserve all testicular function.


In any event here are my lab results.
The first number were prior to trt, and the second number was taken four months into TRT. the number in brackets are the suggested boundary.

TSH : 1.66 >> 0.98 (0.3-5.5)
testos ; 11.9 >> 52.1 (8.4-28)
Prolactin : 201 >> 134 (45-375)
LH : 25.5 >> 0.1 (1.5-9.3)
FSh : 68.9 >> 1.1 (2-20)
Oestradiol:106 >> 73 (0-191)

if anyone would consider my case informally i would be so grateful. i guarentee that I would not abuse, quote or misuse any information you give me, but I would just be really interested to hear what an experienced specialist such as your self has to say as honestly my Harley street endo's haven't a clue and misguided me terribly.

I woul dprobably try a T gel.

HCG will not help you, as yours is severe primary hypogonadism.

thanks for the responce.

i wasnt looking for a way to increase my own production. the hcg would be to preserve my function whilst on trt, and the estrogen only treatment was an attempt to make my own T have a greater effect. Im not looking to stimulate more t production through anti estrogens as i know that wont work, but i was wondering if by blocking estrogen the T i make would work better.

The HCG would be taken with TRT, but i am concerned that in my case it wouldn;t be strong enough to preserve fertility of my own production,

If you are fertile right now, then you are extremely likely to be fertile if you use exogenous testosterone and hCG. But if you want to know for sure, then you need to get fertility tests done.

If I were lying about this stuff, then my wife would kill me.
