It's common for the extractor not to get a good grip on the cartridge rim... cycling the bolt 3 times will not help unless you let the bolt fall hard onto the chambered round thus engaging the extractor. I reload 5.56 and out of spec brass is usually the culprit... or cheap steel case. With that said I sometimes cycle the bolt on a rifle multiple times but always look to verify.

Oh i forgot one
Ive had my badass 4x4 f150 for going on a decade now. Conquered alot of green and some blue trails at some of the most prestigious off road locations in our country. Its my DD and hauls my boat and has brought me back n forth from TX to NY 8 times now. Always some thing in tow.

Whats to blame for its insane durability?
I NEVER EVER turn the key unless the blue oval faces ME. EVER

Ya, this I know. I reload too and not all brass should be reused, even though we do anyways. The 3 x cycling is just my superstition. I give the chamber a visual while I'm doing this to remove all doubt. Cycling a few time's also gives my brain more time to verify that visual, as some days I'm a bit slow (got two small kids, naa mean).

I released the bolt too hard on a worn past it's time semi auto marlin 22 it decided to become full auto at that exact moment and fired about a dozen shots without touching the trigger freaked me the fuck out.lol Always assume a firearm can go off at any moment, and don't shoot shit that's unsafe. Sorry not a superstition ...I don't close any knife someone else opens. Thats about the only superstition I pay any attention too

Shit never even thought of that but I do the exact same thing, I think I was taught it in scouts as a sign of respect too for whatever reason I don't know just always what I've done too

Bad luck on the guy who opened the knife if you close it bad luck on you if you open it and let someone else close it some tell it like your closing the guys life or some shit I'm much further south than you lots of crazy superstitions down here. I don't buy into most but I've seen grown men spin in a circle after hearing an owl hoot in the day

i uhh i dont fuck around with ouija boards, i "smudge" my house with sage once a month to keep the bad spirits out. I dont expect anyone to believe it but there is something dark that has sort of hovered around me all my life, I've seen it on 2 occasions when i was at really low points in my life, now that im doing well i dont think it will come back, but i burn the sage just in case.

Man I did that but with frankincense. Sleeping one night and it felt like someone tried to pull me out the bed by my leg. Probably a spasm but always felt like something was around in that house. Seeing things out the corner of my eye and hearing weird shit. What took the kicker was when I was removing a 9 ft long mirror there was a little upside down cross behind it. After all that was done it doesn't feel like a bad vibe is there anymore. Freaky shit bro.
