So I've been on 80mg ED for a couple of years. Recently I was admitted to the ER after an injury in a car crash. They took my BP and it was 146/92.

If you were me, How much would you increase your telmisartan dosage?

How can people still mess around with peds and not have a blood pressure monitor at home? It's an absolute must. One reading is meaningless. Especially if it's not taken at home when you are completely relaxed.

You are already taking the maximum dosage of telmisartan. Taking more wouldn't reduce your BP.

I have a blood pressure monitor, but they are not as accurate as a stethoscope. My home BP monitor says 125/80

Also that's not the maximum dosage at all lol. I believe it's 160+ mg a day

@Bigboost22 take the average of your home readings over time and that is what it is. one moment in time reading in the ER means is not representative of meaningful data. and yes, telmi can be dosed over 80mg, but it's most effective at bp control at 80mg--improvements are not linear beyond 80mg. it's also one of the mildest ARBs, and there are many other -sartans that are stronger and worth looking at if bp is not sufficiently controlled at 80mg. 80mg is the cut off at which a normal person would just go to a stronger ARB instead of doubling down on more telmi. the lol part silly, you were given sound advice @juicebro

After 80mg usually another option is added like an ACE inhibitor or a mild diuretic

one thing I did that completely fixed my bp was adding some beetroot extract on top of my 80mg telmisartan. That may help you or you may need to reduce to 40mg and add something like a beta blocker. If beetroot extract doesn’t work, I suggest consulting with a doctor

Don’t. Telmisartan isn’t going to give you more benefit at a higher dosage. It is very poor for significant hypertension. Drop to 40mg and add in another more efficacious drug

Which one would you suggest?
