My opinion:

If a bullet pierces a steel plate it leaves one hole no matter the object it's adjacent with one another I would consider it to be one just because it has two sides front or back, enter or exit doesn't make it two

What if the steel plate was 2 inches thick or more.
 
It would be considered a tunnel, given the mechanism in which the "hole" is dug from one point to the other. When you say there's a hole in the ground, you're not saying you'll fall through it from the point you jump in to the other side of the planet, you're saying it goes down to a depth, and then stops. A bullet piercing through a thick dish and coming out the other end would be considered a tunnel

Not initially upon the houses construction, at least in the practices of a few companies I've been aware of in my local area... I'd been hired out by two in the past to help with things from temp services... they build the walls without adding in those sections. Why put up a wall and then have to cut through it and shape, if you don't need to in the first place

I would argue that houses are designed around traffic, either in motion or stationary, so the house is designed around the doorways and built accordingly.

I’ve always thought of a hole as being a 2-dimensional concept. Its a change in perspective to look at it as a hole or one end of a hollow cylinder; 2D vs 3D
