Hypogonadism and hight totalt testosterone

From what I’ve read, it’s possible to have hypogonadism and high totalt T, if SHBG is high. I also read that decreasing SHBG is rarely the solution, since SHBG can increase bc of hypogonadism itself, as the body attempt to ”store” androgens. Since SHBG do store testosterone, it accumulate T overtime thus increasing TT, through high SHBG.

Is this true? It would explain my high serum/total T, high SHBG, high FSH and elevated LH.

Found this study Reappraisal of Testosterone’s Binding in Circulation: Physiological and Clinical Implications

”the male patient with SHBG deficiency had a very low total testosterone level of 4.8 nmol/L (reported normal range, 10 to 30 nmol/L) but a normal free testosterone level (174 pmol/L; normal range of the assay, 120 to 750 pmol/L). Both siblings had normal gonadal development and function. Furthermore, the LH level in the male was not elevated, and he had normal sexual development and normal secondary sex characteristics with normal spermatogenesis, suggesting that free testosterone concentrations, rather than total testosterone concentrations, regulate sexual development and feedback inhibition of gonadotropins.”

174 pmol/L (5 ng/dL) is not high though, I have 12 ng/dL with elevated LH, guess it depend on the individual and the sensitivity of the androgen receptor. 5 ng/dL is very low, read that the healthy free range is 10-30 ng/dL. Maybe SHBG defiency is associated with upregulation of the AR-gene?
