Metformin and stomach pain

My doctor prescribed me 500mg of Metformin IR with every meal. I used to take eat three times a day so that was 1500mg per day. After a week I had a severe stomach ache that I ended up ER.

Doctors checked everything and they said everything was normal as there was some spasm in the smooth muscle likely due to bad food. I dont know what that means.

I tried Metformin ER again this morning and I'm getting same pinching pain at the same location just not as major as I did at the start of this year. Can anyone help me with this? Doctors just tell me to take insulin but I'm not 100% sure of taking insulin just yet.

I'm T2DM and my fasting BG is 110-130 even after doing cardio for months.

What is you diet like? That would be the first place i would start.
Those fasting numbers are not terrible from my perspective but need to be worked on.
If you dr.s don't have any better recommendation then to keep taking MET as prescribed even though you have stated your discomfort to them. You could start with 1/2 tab once a day and work up from there. Ramping up to higher doses seems to work for many with this drug. 1500mg/day seems like a high starting dose to me from my reading.and as it does not work quickly and takes weeks to build up to the desirable effect having with every meal is not needed. I take mine once a day.

It's the metformin, it fucks my stomach up and digestion every single time i try to take it. My friend has experienced the same. He gets indigestion and diarrhea from it. Nothing to do with insulin.

Ask your doctor to prescribe the extended-release version, for some people it helps. By the way 500mg twice a day is more than enough.

He already prescribed it after the ER. But even with that I was feeling the pinch and gas within few hours of taking it.

1.9k to 2.2k calories everyday. Food is white rice, lots of veggies, 3 eggs, salad, and chicken. No food from outside except for once a week when i go to friends house i eat buns with some slight cream applied on it but when i check my blood glucose it is 130-140.

I might be counting white rice calories wrong maybe, but how does one count calories of cooked white rice?

He keeps asking me to take insulin but I don't think I really need it. I want to exhaust my options of fixing high fasting blood glucose with diet or exercise / cardio.

1 cup cooked rice has about 200 calories form most sources i read. And that would not be packed down hard but loose.
People with fasting glucose levels as your have shorter life spans in general. So i would not want my levels there long term.

How many consecutive days have you been able to take the Metformin? It took me about 10 days for the lower GI disturbance to calm down. Basically. Your gut flora goes nuts and then acclimates. Titrating up slowly did help minimize the discomfort.

I got major bloating and disconfort when I started to take metformin at 500mg/day, stopped after a week... but started again after a week break and got no more disconfort. am now at 1g/day and feel fine. (extended release)

I've heard this from others as well so there might be something to it...

but as others said start low 500mg or even less

yes extenden release 500/1000mg with last meal , it's great for example when you take all GH dose pre bed , in the morning your BG is perfect , now i use 5/6 UI GH one shot pre bed and in the morning my BG is perfect

T2D and eating carbs dont go together well. Consider a low carb diet and staying there for, um, the rest of your life. You might even be able to come off the Metformin.

In the meantime, its a good idea to start supplementing with B12 (methyl cobalamin or adenosyl cobalamin).

Metformin is known to push B12 levels down.

I got $5 says the doctor who prescribed Metformin for you didnt tell you this.

Does the same thing to me. Doesn't matter what I eat.

I'd research your particular brand and batches on the FDA recall site, historically, whatever suits ya... They can't seem to manufacture Metformin without high levels of NDMA (N-nitrosodimethylamine) contamination these days -which is not good. There's actually a lot more medications out there that this stuff seems to be finding its way into somehow. I think they say its a side effect of poor manufacturing process.

"NDMA overexposure can cause side effects including:
  • Headache
  • Fever
  • Nausea
  • Jaundice
  • Vomiting
  • Abdominal cramps
  • Enlarged liver
  • Dizziness
  • Reduced liver, kidney and lung function"
