Need help knowing which direction to push.

Little bit of a background I’m 20 years old, have lost 140 pounds and am currently on medically prescribed TRT.

Stats :
5’7
168
I would assume 15-20% bf not a good judge myself.

I have been dieting around 3ish years with periods of for maintaining, but I really want to put on some decent muscle. I know a lot of people on here are experienced and I’ve been kinda stuck. Should I push to get leaner, or would starting adding more food be the better approach? At this point dieting has become second nature so I’m not worried about getting fat again, but I’m also weary enough to not dive in headfirst and sabotage myself.

I think you can aim for a gaining phase. Start gently increasing calories, 10% per day over maintenance. It will also be a nice break for your system after all the weight loss. The goal is to get your body eating as much calories as possible without gaining too much fat. It will make the next cutting phase easier.

I'd start a slow surplus. It seems like you're already decently lean based on the photos. If you feel like training progress has stalled, I think clean bulk is the move.
