Hello forum,
I have a very strange problem, and have spent the last 6 months of my life trying to figure out whats gone wrong. I'll try to put it out as fast and complete as I can. I am 24, around 8% bodyfat, weigh 160lb/ 5/10'' lift weights regularly, and am in very physical condition. I have never done steroids or used any strange supplements before. Before "the problem" started, I had a RAGING sex drive and never had an ED problem in my life. I was like a normal person for me age, I guess. I couldn't get enough sex, and slight fantasies about girls would turn me on in a flash. Around 6 months ago, I masturbated for a long session, and the next day I feel a little off, but think nothing of it. As the days go by, I see my penis has shrunk down some, sort of the look when you get out of a pool, and and isnt responsive/ and I don't have any SEX DRIVE ever. I lost all spontanous/psychogenic erectile ability. I wait about 1.5 months and see a urologist, he looks at me, says heres some Cialis, it should get better with time. I take this regularly, and as the months goes on, my libido stays very low, with one temporary peak that lasted about 2 days. I felt horny and normal again, and my penile skin felt sensitive. I masturbated like normal, and it felt great. That peak was about 2 months ago.
As soon as it came, it was gone again, and I am left with no libido, and minimal penile sensitivity, and a penis that doesn't look or feel the same anymore. I have a ton of new visible veins in the penis, one that seems varicose, that I am 100% positive were never visible before "the problem". I have also lost flaccid girth, but im hoping its reversible somehow.
Here is the final kicker, that maybe will help some of you guys with hormone experience give me some possibilities. I got some bloodwork, and my testosterone free and total is high. Here is the results of everything.
Testosterone Free and Total, LC/MS/MS Range
Total Testosterone 1075 250-1100 ng/dL
Free Testosterone % 1.88 1.10-2.80%
Free Testosterone 202.1 H 35.0-155.0 pg/mL

Thyroid
T3 Uptake 36H 22-35%
T4 Total 7.1 4.5 - 12.5 mcg/dL
Free T4 index (T7) 2.6 1.4-3.8
TSH 2.84 .40 - 5.50 mIU/L

The thyroid seems pretty normal, but my total testosterone is high and my free testosterone is over range. Yet, I have no desire for sex, the skin on my penis feels like the skin on my arm. I can get an erection with manual stimulation, but i'm not aroused at all, even by porn, so it doesn't feel that pleasurable and it doesn't last. Its so bizarre to be able to look at porn, and what once was extremely erotic, just seem like watching something only minimally arousing. Orgasm doesn't feel anywhere as pleasurable either. What should I do from here? Does this sound like too much E2 or too little E2, or maybe too much prolactin? I really want to get my life back to normal, and I can't think of anything else this could be. Aside from this issue, I have no depression issues, and am staying positive right now, cause I know I can beat this. If you rule out depression, and diabetes, what else could cause this. Also, I do wakeup with morning wood on occasion, but when I do, I sometimes don't even realize it, cause there is very little sensation from downstairs, and its never stays up for long after waking, like it used to. That tickly electric sensation has been gone for about 5.5 months. I sincerely appreciate everyones time!

"Dude I shit you not! Since taking these damn steriods, I've lost the 9.5" between my legs! A damn 7" now! Can't get my penis to stay hard at all! By the time I'm through with AAS my penis will be about 4"'long. SHIT!" I'm not really worried about it, except when I meet a chick, even with the sex aids, soft rod! Well, going to mash some grapes, sing some hymns, do some chanting, and read The Bible, until this hell is over!."

Well your testosterone is certainly not the cause.. Are you experiencing any other symptoms, like fatigue etc. Having a TSH over 2 is suspect and may indicate thyriod problems.

Well your test is good.
Do these test at Quest if possible.

Estradiol, Free, LC/MS/MS (36169X)
Estrogens, Fractionated, LC/MS/MS (36742X)
Estrogen, Total, Serum (439X)
Dihydrotestosterone, Free, Serum (36168X)
DHEA sulfate
Prolactin
Progesterone, LC/MS/MS
Pregnenolone
FreeT3
FreeT4

My first guess would be as mentioned above would be a thyroid problem. Your TSH is high and your total T4 is also low. Get the other tests JanSz listed @ Quest. (Total T3, Total T4, Free T4, Free T3, TSH) Then you will know for sure. I can tell you this...my TSH and total T4 were almost exactly what yours are before I was diagnosed with hypothyroidism. After looking at Free T3 & Free T4 it turns out I was only producing a little more half the thyroid hormone my body actually needs. I also can relate to those symptoms big time...Even with my total T in the mid 600's I felt almost ZERO libido. Nothing will kill you sex life quicker than hypothyroidism! I am getting better slowly but surely now that I am being treated for this.

Recless, did you suffer the shrinkage problem too? It seems a common finding on these boards. How are thing's now libido/ED wise? What protocol are you doing?

Hey Guys,
Thanks for the replies so far. I will definately try to get some more tests from the doctors, and hope I can diagnose this problem. Its just hard pushing these mechanical doctors around, who look at the results, and say, "well you aren't out of range on anything here". I have a feeling it might be hypothyroidism as well. Before it happened, I was always really depressed, but never bothered my libido or sex drive. I was ultra horny. But I would always feel the best around 3am, and I would sleep till around 2pm everyday, getting at least 10 hours, and still feel exhausted. No doctor every diagnosed me, but I felt like it was either that or chronic fatigue. Is there anything I can do naturally in the mean time if it is hypothyroidism to help it heal naturally, or is this a RX only battle?

Hyoerprolactemia.
Check your prolactin levels. Your Sx fit perfectly with a prolactin secreting tumor
Keep us posted please

I just read about a guy here that has been sick for yrs and just found out what was wrong. You show sines of his problem. High T levels and possable Thyroid problems.

I have hashimotos thyroid disease. I agree thyroid problems. Too low of a thyroid definately hard on the erection

Guys, this is probably a stupid question but how do you know if the veins are vericose? I am also in good shape but have had the same symptoms for a LONG time. As you can see in my recent post I also have high T but didnt have the thyroid test done...even though I plan on doing it soon. Thanks.

Sounds like you may have adrenal problems. Read the following link and do the questionnaire mentioned in the link and post the results.

Do the following labs.

Free T3
Free T4
Prolactin
Estradiol
DHEA Sulfate

My 2 cents. First thoughts after reading this post are that you definitely have a physical problem less complex than hormones. I was thinking potential blood clot in the stream??? You did not mention skin color. Does everything look normal in color. Varicose veins are tipically stretched veins that occur due to the backward valve in the many sections of a vein starting to fail. When this valve fails, the weight of the blood increases by how ever many sections of vein the valve failure has occurred in. They are also usually in the legs when the most circulatory stress can be found. As blood has to travel back up, of sitting you may cut off return blood flow increasing the pressure on the veins as the blood tries to return to the heart. You may want to request some bloodwork to determine clotting factors. I think there is one that they give people who may be having a heart attack that can determine what your current clotting situation is. I am not sure exactly. Just my thoughts.

I can relate to the stories above, I also would say that my balls have shrunk :( I am taking 500iu HCG per week and find it very strange that I have this penile shrinkage as well as my balls shrinkage. Can anyone help ???

Hmm I know exactly how you feel OP. I'm gonna keep up with this thread even though I think my problem is hormone related, cos my free T is below normal range and my total T is near the low end. And as it happens i'm only 25 and having been working out for the last 7 yrs of my life and had this no libido problem for the last 2-3 yrs atleast :/

SEXUAL EXHAUSTION IS YOUR DIAGNOSIS.

you need to bring down prostaglandin e2
raise endorphins,dynorphins etc etc
bring down prolactin and noradrenaline levels
raise dopamine
check your androstanediol glucuronide activity (if too low or too high)
check and control e2 levels
control cortisol levels

refrain from masterbation or sex for a few weeks to months, and limit your stimulants in your diet (coffee, alcohol, sexual stimulation etc etc)

try to get more omega-3 in your diet
however if you androstanediol-glucuronide shows low conversion rate of T to dht then you are in more complicated problems
