Hello everyone! I am in my twenties, and I suffer from low t. Unfortunately in my (northern europe) country I can only get prescribed really bad testosterone ( oral, and the cream that you need to put on your skin in specific areas.) I have had appointments with my family doctor, and done various blood tests to check if I only have low t during the winter season. I went to an endocrinologist - Turns out I just have low T, I am looking for advice on how to approach this issue. I have been eating clean, going to the gym with a personal trainer for 6+ months, and I feel like shit most of the time regardless of what I try to do. I was fat during my teenage years, but during covid I lost all of the excess fat, and looked somewhat good. Skip to today, and I feel tired every single day, I am in a bad mood most of the time, and can't even do basic tasks. Currently I am skinny fat, and I believe I don't feel good, and can't achieve the results I want due to low T. Is there anything else I should try do to before putting needles in my ass cheeks? Share your bloods first.

Otherwise, what endo's usually try is taking a SERM. If the problem isn't in totally unresponsive gonads.
 
Ditto what @Jin23 said, let's see some bloodwork. I'm not sure what the protocol in N EU is, but in the states endo's will typically start with something like clomiphene before jumping straight to Testosterone (cream, patch, or otherwise).
 
Low T at 20 years old sounds a bit odd.

Have you cycled before? Taken pro hormones? You must have done something to crash your natural test levels.

In terms of solutions, like @Jin23 mentioned, a SERM would be a good first step.
Nolvadex @ 20-40mg for 8 weeks
and/or
clomid @ 50-100mg for 8 weeks
 
I would recommend going to a urologist and schedule a testicular doppler ultrasound, there's a chance you have a varicocele that doesn't give you any signs. Most varicoceles doesn't screw much with the test, just a slight decrease, but there are cases that show a significant decrease in test levels. Before you start TRT just make sure you need it because it's a lifetime commitment and you're still young, but if after all trt is the only way feel free to start asap, ditch the gels and patches and take matters in your hands. Find a source and start doing it your own way if doctor does not support you. Or you can start with the gel, take that prescription that allows you to use testosterone legitimately, leave the gel aside and just use oil based test to pin. Just a thought.

You say you are skinny fat. That might be part of the problem.

What is your current body composition?
That's the most interesting thing, I was quite muscular a couple of months ago. But then I got a bit sloppy with my diet, and workouts. While I was somewhat consistent, I noticed an increase in weight, even though I was on a caloric deficit at the time for 3-6 months. 250kcal deficit to be exact, I'm at 18ish bf%, not quite sure what you meant by body composition, so I hope I answered your question correctly.
 
One important thing to note, my endo said that my low testosterone could be impacted by season. I'm not sure what he meant by that, but I believe it has something to do with seasonal depression (which I have been experiencing since 17.) Every time winter starts I feel like utter shit, and I am extremely demotivated.

So you're saying you have low T, but haven't gotten bloodwork yet. How do you know its low T and not just depression, especially when you're saying you have seasonal depression

I have done bloodwork brother, it's just not the most recent one. I had bloodwork done 2 months ago, and since I will be doing another one on Monday, it would be more than reasonable to show you guys the most recent results of the bloodwork.

I had a 147 ng/dL test level at 23 years old. It happens. My dad had very low testosterone his whole life as well. Fortunately I’m in the US so getting test prescribed is very easy. After being put on trt I’ve been cycling and haven’t actually used any legal testosterone since then, so UGLs are always an option if you exhaust every other option. Just know that once you’re on TRT it’s likely forever

If @readalot mentioned all of the necessary blood work related things I need to check, I'll ask my family doctor on monday to check the boxes (hopefully I have enough money for it, because checking test, and other stuff costs money here), and get those done. Vitamin D is at 58,4 if it's relevant.

My endo told me that something wasn't adding up, not sure what he meant by that. That's why I will be getting a new blood test in a different hospital.

TSH (last line) is euthyroid.

Not necessary (according to most Endos) but you could get free T4 and free T3 so you have a baseline.

Your TT result looks like it came from immunoassay with that funky reference range. Bottom of that ref range is really low. So if it were me I would try to get a TT number using LCMS assay where reasonable reference range is 300 to 1000 ng/dl. With that you can use Vermeulen tool to calculate FT...


Or if you want measurement (may be $$$) get the Doc to order an equilibrium dialysis FT assay. Eugonadal ref range is 10 to 25 ng/dl.

LH/FSH/E2 and the rest above will give you decent baseline bloodwork.

You may be suffering from a crappy TT assay instead of low TT/FT. Good luck.

Edit: the upper reference range on the SHBG assay is really high.
