Thoughts on E2 with ankle swelling? Blood tests attached.

back story: 34 year old male that let myself go after the military, got fat and up to 270 pounds. After cutting all the weight off (with the help of TIRZ. & recently RETA) i got down to 190 lbs and got labs. As you can see i had a test level of 168. Despite this, i was luckily able to retain a good amount of muscle in this cut but i decided to start trt. I know i started too high @ 200 M TOTAL injected 2x weekly @ 100 per injection . After the first 3 weeks i started noticing ankle edema on both sides. Without being able to get ahold of my DR i panicked and went to the ER where i had an EKG and all kinds of testing which ruled out anything heart related.
After all of this, 8 weeks later into starting TRT, i finally got my labs back and my E2 Is at a 78. The numbers on the labs are taken at trough, injecting every 3.5 days and blood was drawn fasted at the 3.5 day mark prior to next injection. I’m assuming this likely is the cause of this edema?
My question is this: I have tested primo100 at the house & anastrozole. Should i take a small AI dose, like 0.25 2 x a week and re test again in 6 weeks and see if the bloating edema subside? Should i try 100 MG primo alongside the test cyp and see where that puts me?

current labs after 8 weeks on dose: test: 1119 ultra sens. E2: 78

Aside from the ankle swelling, i feel pretty damn good. In all reality, i need to cut another 20 pounds. 190 is good, but 170 would be a lot better.

olid progress on the weight loss, man. Dropping from 270 to 190 is a massive win.

Regarding the edema: it’s almost certainly that E2 spike combined with the jump in sodium retention you get when starting TRT. 1100+ at trough is a high ceiling, so your peak is likely way up there.

If I were you, I’d keep it simple before adding more compounds. Start with the Anastrozole @ 0.25mg twice a week (on injection days) and see how your ankles look in 10-14 days. Adding Primo now might complicate things since you’re still dialed in on your cruise dose.

Drop those last 20 lbs and your aromatization will likely settle down anyway. Keep at it!

I personally take care of this with eplerenone (really effective if it's aldosterone -related), and indiapamide XR. It's doesn't cause a strong diuresis but you pee more sodium all day and I'm leaner.
I take 1.75g test C, 1,2g EQ and 11UI of GH, no edema, sometimes a little bit when I stand up all day, but it goes away during the night.

running them doses i’m assuming your pretty experienced in this world, with that being said, is it just part of the game for some people to get some minor edema like i have? it’s crazy because i feel on top of the world other than that, no other high e2 symptoms at all… well accept for the increased anxiety that made me go to the er in the beginning because i thought i was having heart failure lol but after confirmation testing, that anxiety has lessened a lot.

Diuretics usually afraid people but it's a very large pharmacological class. Saying their dangerous is like saying fat are dangerous. Some are for sure, but some are pretty beneficial too.

Some people like us can easily have edema, my mother and grandfather had edema too and they were normal people (no sport, no ped whatsoever).

The point is to control aldosterone activity and to promote the loss of sodium without causing hypovolemia (meaning causing too much loss of water in the blood). This is pretty bad for the kidneys and can really really worsen water retention by spiking anti diuretic hormone as a stress signal.

Monitoring electrolytes is essential, I retain potassium easily (eplerenone), calcium too (indapamide), and a little loss of magnesium (indapamide).
