Don't want to become a cock rider doc, but I think you succinctly summarise the way most of us feel.

For me the watershed was CND's pancreas op, and while I don't want to speculate too much, he's become way more delusional since.

I don't have any skin in the game, and really worry about his state of mind. Perhaps someone like @Michael Scally MD can gently persuade him to get help, or at least wind it back a bit.
