What Is Normal and Who Qualifies? Validated Questionnaires for Penile Size Assessment and Body Dysmorphic Disorder

The Diagnostic and Statistical Manual of Mental Disorders describes body dysmorphic disorder (BDD) as the preoccupation with imagined imperfections that cause severe anxiety for the patient.

A subset of men with BDD includes those who are consumed with anxiety and worry regarding their genitalia being inadequate and/or too small, despite clinical measurements confirming otherwise.

Patients who hold this false belief and experience significant impairments to daily living are considered to have penile dysmorphic disorder (PDD). Although the rate of PDD is currently unknown, some studies report a 1%–3% prevalence rate of BDD in the general population.

This increases when studying patient populations of specialized esthetic clinics. General cosmetic surgery and rhinoplasty surgery clinics, for example, report the greatest rates of BDD or up to 13.2% and 20.1%, respectively.

It stands to reason that the rates of PDD are likely also increased in urologic clinics that specialize in men's health and penile augmentation.

Penile Size Dissatisfaction

Background Men concerned about their penis size often consult professionals working in urology, andrology, surgery, and sexual medicine.

Aim To inform professionals in the sexual medicine field about small penis syndrome as a clinical syndrome and to provide recommendations for treatment.

Methods This was an overview of the existing literature combined with our extensive clinical experience. Results Small penis syndrome is a syndrome with psychiatric comorbidities and social consequences that impair life. Men with these concerns tend to be susceptible for treatment that is not evidence based and potentially harmful.

Clinical Implications Treatment of men with concerns about penis size should start with a thorough biopsychosocial assessment, followed by extensive psychoeducation, counselling, and psychological interventions, even if surgery is being considered.

Strengths & Limitations The strength of this study is the concise overview of the existing literature combined with clinical experience which leads to important recommendations. Limitation is that this is not a systematic review.

Conclusion Complaints about penis size should be taken seriously, and a thorough biopsychosocial and multidisciplinary assessment is required.

Giant snow penis appears in Swedish town

A three-metre high snow penis appeared at a roundabout just outside Borås after heavy snowfall on Tuesday, causing both giggles and safety concerns.

The roundabout where someone took the opportunity to build the huge creation, is located at the exit from highway 40 towards Dalsjöfors and Äspered.

According to motorist Perra Larsson, it has become something of a tradition for such a snow sculpture to appear at the roundabout.

"This is the fifth year it has been there. And I think it's a fun thing," Perra Larsson told the newspaper Borås Tidning.

Helena Alçenius, the CEO of Borås TME, a municipal company which markets Borås laughed when she saw pictures of the creation.

"I am a little impressed, because it is not something that you build in the blink of an eye and it must be site-built. It looks well made, I hope they had fun when they did it", she told Göteborgs-Posten.

Penile Length Measurement: Methodological Challenges and Recommendations, a Systematic Review

Background - Penile length measurement techniques vary widely in published studies leading to inaccurate and nonstandardized measurements.

Aim - To review the methodology used to report data in studies evaluating penile length and provide a detailed recommendation in conducting future high-quality research.

Methods - The MEDLINE database was searched for randomized clinical trials and open-label prospective or retrospective studies.

Outcomes - The panel reviewed the modality of data reporting on these specific areas: patients’ age and assessment, patient position, type of measurement instrument used, penile length technique description, examination conditions, and actual examiner.

Results - Overall, 70 studies investigating penile length were selected; among these, 72.85% included at least 50 patients: 16 prospective studies, 5 randomized clinical trials, and 49 retrospective cross-sectional studies. Amongst all studies, 90% reported to measure penile length by health care practitioners in clinical settings. Penile length was assessed in all 70 studies, whereas penile girth was measured in 57.14% of patients. A semi-rigid ruler was the most commonly used measurement aid to assess penile length/girth in 62.86% of studies. Penile measurements were reportedly obtained: (i) stretched state, 60%; (ii) flaccid state only, 52.68%; and (iii) during erection, 27.43%. All studies investigating the penile length in an erect state were simultaneously assessing penile length in the flaccid state. About 90% of studies investigated penile length in adults, whereas 10% were conducted in adolescents.

Clinical Implications - The use of shared methodology to assess penile length in both adults and adolescents allows more accurate and standardized measurements.

Strength & Limitations - A systematic review of the published literature allowed proper data interpretation in order to provide accurate recommendations. Main limitations of the study relied on a relatively limited number of databases for the identification of potentially eligible studies.

Conclusion - The methodology used in studies measuring penile length should be precise and standardized in order to provide accurate data to both clinicians and researchers.
