Melatonin as a sleep-promoting agent has been tried in a small non-homogenous group of elderly patients with primary insomnia (3 mg p.o. for 21 days) associated with dementia or depression. Seven out of ten dementia patients having sleep disorders treated with melatonin (3 mg p.o. at bed time) showed a significant decrease in sundowning and reduced variability of sleep onset time [263]. In another study, administration of 6 mg of melatonin to 10 individuals with mild cognitive impairment improved sleep, mood, and memory [264]. Similar observations were made by other groups, too. Seven AD patients who exhibited irregular sleep-wake cycles, treated with 6 mg for 4 weeks, showed a significantly reduced percentage of nighttime activity compared to a placebo group [49]. The efficacy of 3 mg melatonin/day at bedtime in improving the sleep and alleviating sundowning was shown in 11 elderly AD patients [265] and in 7 patients of another study [266]. Long-term administration of melatonin in the dose of 6–9 mg to 14 AD patients with sleep disorders and sundowning agitation for a period of 2–3 years improved sleep quality [267]. Sundowning, diagnosed clinically in all patients examined was no longer detectable in 12 patients. Another study on 45 AD patients with sleep disturbances, in which 6 mg of melatonin was given daily for 4 months, confirmed sleep improvement and suppression of sundowning [268]. Along with these ameliorations, which can already be seen as an important improvement, also with regard to the efforts of a caregiver, the evolution of cognitive alterations in melatonin receiving patients seemed to be halted in several individuals, as compared to AD patients not receiving melatonin.

I've tried to read-up on the school of thought that sees Alzheimer's, Parkinson's, and ALS as the same disease with your genetics determining which one might land on you. There's a lot of smart people on this board so maybe this isnt news to some of you.

There's a book on the subject I havent got around to reading yet but maybe someone here will read it before I do and get back to us.

I've tried to read-up on the school of thought that sees Alzheimer's, Parkinson's, and ALS as the same disease with your genetics determining which one might land on you. There's a lot of smart people on this board so maybe this isnt news to some of you.

There's a book on the subject I havent got around to reading yet but maybe someone here will read it before I do and get back to us.

Havent read the book but from the title would assume it refers to the neurotransmiter glutamate. Memantine ,10mg/2xd, seems to hold it in check in the hippocampus.

Yep. The idea is that glutamate neurotransmitter receptor sporting cells can be driven to energy failure under certain conditions. Adding ketones as an alternative to glucose appears to supply energy to allow both survival and improved function of neurons which are doing very badly on glucose.

This story has been on the internet for a while now and it seems some people with a certain gene will get some relief with MCT oil or coconut oil (contains MCTs) that then get converted in the liver to ketones:

And ketogenic diets have been used to partially REVERSE Parkinson's:

It appears that if you feed dopaminergic brain cells on ketone bodies they stop dying, and maybe the sick-but-not-yet-dead ones recover. The brain likes ketone bodies.

I'm still on the early part of the learning curve with all this...so maybe these approaches have since fizzled out??

I think it is a viable approach as near as I can determine.
Really glad to find someone interested in this who also has the ability to comprehend the vast amount of data on the subject.
I`ve been directing my own health care for several chronic issues for a few years now with the cooperation of several doctors.
I started studying MCI and AD maybe a year ago and have taken the responsibility for my wifes treatment. My dx of her at this time is MCI. I dont want it to progress any further.
My first choice of drugs was Donepizil which addresses the low acetylcholine levels in the brain. Standard treatment really in ~70% of suspected cases of AD.
I have a few other ideas that I want to run by the neuro today.
 
My 82 year-old mother has MCI/AD, though no formal diagnosis. She use an Exelon patch for a few months until it lost effectiveness.

I`m sorry to know that. Are her Sx mild,moderate or severe?
Aricept has been approved for 23mg/d. Maybe that in conjunction with other treatments might help. The medical profession moves way too slowly at times.

I've written to my legislators...they don't care. We can vote out the incumbents...but the new ones get as crooked as the old ones just as fast as we can replace them.

I don't think anything is ever going to change unless individuals finally stand up.

Sometimes things have to get bad worse befor they get better.
I said that when GWB got elected the first and second times and I`m still saying it.
The recent events in the middle east are promising. I kinda believe in a world consciousness. We are here because of the French revolution ya know. Consciousness IS life as I see it and It spreads by its nature. There are those who want all of lifes bounty for themselves and have a long histroy of being in control. But if you look at the the elolvution of mankind you will see it consistantly weakening over time. The printing press was big. I think the internet will prove to be an even greater equalizer.

Me too. But I think as soon as anything started to happen like what is happening in the middle east the internet kill switch is the first thing that would get hit.

I would say mild. She is 90% able to take care of herself. The main problem is short-term forgetfulness. Two days on coconut oil (3 tblspns/day) and there seems to be some improvement but there's no way to know for sure just yet. As soon as the MCT oil arrives we'll switch to that.
