Fiancee going through perimenopause. Advice?

Estrogen is restored to the lowest levels at which a woman feels better to mitigate risk. Modern HRT doesn’t go by levels because what works for one person may not work for another. And I guarantee you no woman wants as much estrogen as they had at 20. The side effects of even natural estrogen are miserable. For some women low level systemic estrogen replacement may be enough for vaginal atrophy. For most it is not.
I’m in a fb group for hormone replacement and there’s a large majority of women who feel best at higher levels and didn’t have symptom relief until then. I can’t recall but I know there are minimum levels it needs to be at for protective benefits.
 
Most women don't like the pill. It is also made from horse urine so there are some ethical concerns. Its also just plain fucking gross
That’s Premarin and not widely used anymore. I believe the preferred oral estrogen is derived from the same source as the transdermal E. I haven’t looked into it, but I’ve seen claims that oral estrogen offers the most benefits and some of the lady drs I follow prefer it post menopause for that reason. Idk, it’s something I’ll look into later. 2 drs I can think of the put their own moms on it, so I figure it must be worth it. I did see info that while there is risk, the numbers are really low, and go down with time. Something like from a baseline of 2% to 4%, I am making numbers up, but you get the idea

Varies. I was told standard starting dose is .05 for the transdermal patch and 100mg oral micronized progesterone. Anecdotally I haven’t seen any woman besides Nunya here have symptom relief with a lower dose, or they have symptoms return quickly. I know my practitioner is willing to start at the lower dose though to avoid potential side effects bc the body can sometimes go nuts with exogenous hormones are added. The first gyn who treated me based solely on labs doubled my E even tho I had no symptoms and it caused my cycle to come really early and it was basically a wk of hemorrhaging, followed by another wk of spotting. The 100mg of progesterone is just standard regardless, typically increases when the E is raised.

Most drs stick or transdermal patch or oral estrogen. The oral is synthetic and has increased risks. There are online clinics that will do injections. Defy is a popular one.
I’m not sure I’ve seen any benefit from the estrogen patch I started (last week). The vaginal estrogen for SURE I’ve seen a difference in just a couple weeks and the progesterone improved my sleep after just a couple doses. I’m sure things will change as this process progresses though, and I fully expect to need to go up on the estrogen.

For me I notice a difference even within hours if I don't change my patch. But I will say I did have to increase it to 0.1 before I saw significant relief. But I mentioned before I also notice certain patches absorb like shit! Just something to think about.
 
What is the transdermal? Is it just something you rub on your vagina? I wonder if I can find that with some of the sources here . Her vagina is definitely not atrophied I know that.
This is something that happens over time. It may not be there yet but its a concern. Have her use an online service like myalloy or Midi if she doesn't want to go to her doctor. Im not trying to be a dick but trying to treat symptoms when you don't know much about menopause and options is going to lead to disaster. Once she has some idea from a knowledgeable source THEN you can try and find it through other means if you want.

Either means your dose is too low or you’re not as much of a raging bitch as I was LOL I’ve seen some women say it took them longer to feel like it was working. I felt better within the first week and couldn’t believe my husband was in fact not the source of all my problems ha The vaginal estrogen helped so much, and almost immediately. I was actually shocked. I feel like the estrogen cream dealer and am always pushing it on my friends

It’s crazy to me that nobody talks about this stuff or even knows all the stuff that comes out of nowhere is hormonal. I’ve got teen sons and they know more about hormones than they ever wanted. Idgaf, now they’ll know and their women won’t suffer later trying to figure out wtf is happening.
 
Edit: I did start off on test-p and I preferred the way I felt but I was tired of frequent inj. so I switched to testC 5mg 2 times a week. Between the test, peptides and placenta inj. I was starting to feel like a pincushion.

I got Plamon last month but haven’t used it sub q yet since I knew I was going to need to adjust my hormones and have thyroid rechecked. Wasn’t sure if it would affect labs or not. I chose the Plamon bc it can be microneedled and used sub q.

I’m pretty early in the process and suspect my estrogen hasn’t dropped as much as my progesterone…YET. But I definitely noticed that as I’ve lost body fat the symptoms of low estrogen have been more pronounced.

I’m assuming she was talking about injectable placenta, which I know sounds crazy. It’s a legit medical treatment in Japan and Korea. I sourced mine from a Korean vendor, visage medi. There are some other places, found them in a skincare group. Lmk if you’re interested and I’ll msg the info. I think you need to contact them and create an account to see all the offerings. There’s a dosing guide specific for meno relief, but it also has other regenerative and healing properties, especially for the liver and ligaments. Laennec is the original, but I’ve heard has potential to be more pinchy. Melsmon is the other one I’m familiar with, I went with the Plamon bc it’s the least expensive to try and see how it goes.
