Help - Severe gynecomastia

Severe gynecomastia --> since age 12

Age 18 --> lost about 50 pounds, gynecomastia still present and also suddenly lost about 50% of pubic hair

Freaking out I went to an endocrinologist who ran every thing there is to run including a PCR (normal XY), full hormone panel- all returned normal and I was discharged. She basically dismissed everything I said.

7 years later, nothing has changed and I decided to repeat labs on my own. I got the following results:

LH 2.76 (0.57-12.07 mIU/ml)
FSH: 1.19 (1.37-13.588 mIU/ml)
Estradiol: 34 (11-44 pg/ml)
Progesterone: 0.3
(<0.1-0.2 ng/ml)
Testosterone, Total: 5.29 (1.56-8.77 ng/ml)
Testosterone, Free: 14.74 (8.80-27.00 pg/ml)

I have many, many other symptoms of low T.

How can I have CLEAR hypogonadism and yet normal testosterone?

Can I do anything to lower progesterone since that may be the cause of the problem?

Suggestions? By the way, still awaiting new Prolactin and Cortisol.
 
I think it is said 80% of males have a form of this. (just do a google search on it).
of course they would dismiss your thoughts of low t. Based on the results its not low. Estrogen is a little high but within range. If is just fat maybe it will go away if you lose more weight maybe not. It could be in a spot that no matter what weight you lose it will always be there. In that case surgery. If it is hard lump you could try some letrozol or surgery.
Progesterone canbe the cause it is also said igf- 1, LH, prolactin, can do it also. you coudmaybe try some arimdex talk to your doc about it. That could be giving you the low t symptons.

Hi. I’m a beginner. I have a question, my thigh hurts where I gave myself my first testosterone e (300mg/week in 2 shots) injection. What was the problem?
 
Delts or glutes, imo. I prefer the former with a .5" syringe.
My urologist wants a 22g x 1 injection in the thigh, but from what I’ve read on MESO, there’s no real benefit over using a smaller gauge in the delts. Do you use a 27g with your .5” needle? I wonder why these doctors don’t just go for the delts?!

I’m also struggling to get my TRT covered by insurance, which is how I ended up here in the first place. Finding a knowledgeable and supportive doctor has been a challenge… l’m about ready to give up on the doctors/insurance route altogether and just handle this myself. I appreciate all the information you and others provide here.
 
My urologist wants a 22g x 1 injection in the thigh, but from what I’ve read on MESO, there’s no real benefit over using a smaller gauge in the delts. Do you use a 27g with your .5” needle? I wonder why these doctors don’t just go for the delts?!

I’m also struggling to get my TRT covered by insurance, which is how I ended up here in the first place. Finding a knowledgeable and supportive doctor has been a challenge… l’m about ready to give up on the doctors/insurance route altogether and just handle this myself. I appreciate all the information you and others provide here.

No need for them to update from the 1960's standard he was taught during the 30 minutes spent on TRT in med school.

You can manage this yourself.

If you're going to pay out of pocket may as well use a men's clinic.

If not, then just get some quality test-cyp, good BD syringes, and handle your own blood tests using something like ultalabtests and the info here. You can even post the results and very experienced members can interpret them for you.

Tuck away a vial of prescribed test (even empty) to make international travel easier if necessary.

You should be able to crash your own testosterone levels to the point that insurance should cover you. It'll suck, but once you get the Dx of hypogonadism and your Rx for testosterone enanthate/cypionate, you'll be on your way. Even if you end up taking care of things yourself, having a script on hand can be invaluable.

My PCP initially wrote my script but had the dosing at 100mg every 2 weeks. On the follow up he refused to adjust or raise the dosing so I went with a HRT clinic. The cost is about the same if you use a high deductible plan and find the right clinic. I just use my HSA to cover it.

For the syringes you can use a 31 gauge insulin syringe. If you put 1mL in there, it’s going to be aggravating to push in but leaves minimal marks.

Helix HRT in Las Vegas. Give them a call and run the numbers. They can go up to 400mg/week.

Easiest way is to run exogenous testosterone for a few weeks and stop. That should suppress endogenous testosterone production, and you just wait for the exogenous testosterone to leave your system - shorter esters are better for this.

Two weeks of test prop 50mg EOD followed by 2-3 weeks of cold turkey should have you below the clinical threshold for a diagnosis of hypogonadism and a prescription for TRT.
