I'm unaware of ANY drug or "analog" whose effects on IGF levels are not mediated thru hepatic ENDOCRINE activity.

Your "autocrine" statement implies the physiologic effects of IGF can be mediated by much more local phenomenon at the cellular level.

Correct, in fact there are feedbacks in place that suggest elevated endocrine IGF-1 levels might even act as a negative regulator upon autocrine IGF mRNA expression.

And there are plenty of trials out there which show that autocrine mRNA expression can be elevated in a manner completely independent of endocrine IGF-1.
