Smoking marijuana modestly reduced pain and other symptoms of chronic neuropathic pain, results of a small randomized, placebo-controlled trial showed.

The most potent dose used reduced average daily pain scores by 0.7 points on an 11-point scale (5.4 versus 6.1 with placebo, 95% confidence interval for difference 0.02 to 1.4), according to Mark A. Ware, MBBS, of McGill University in Montreal, and colleagues.

Those who smoked weed with 9.4% of the active ingredient tetrahydrocannabinol (THC) also reported sleeping better, the researchers reported online in CMAJ.

These results are important in light of the fact that patients who hear about pain relief from ongoing publicity about medical marijuana have had only a "trickle" of evidence to prove it, explained Henry J. McQuay, DM, of Oxford University, in an accompanying editorial.

"If medical cannabis is not available where a patient lives, then obtaining it will take the patient outside of the law, often for the first time in his or her life," he wrote. "Good evidence would at least buttress that decision."

These quality results along with three other trials of smoked cannabis for neuropathic pain do support an analgesic effect that, "though not great, may be of use to some patients," McQuay concluded.

This study does offer hope since few drugs have proven effective in these patients, commented Steven P. Cohen, MD, who as director of pain research at Walter Reed Army Medical Center in Washington, D.C., sees chronic pain in most of his patients with major war injuries.

However, the magnitude of the pain relief from smoking marijuana was less than expected compared with those few effective drugs, Cohen noted in an e-mail toMedPage Today and ABC News.

"When considered in the context of the higher incidence of minor and serious side effects with medical marijuana, cannabinoids should remain a third or fourth line drug for neuropathic pain," he wrote.

A bigger concern remains the "delivery system," which is substantially worse than tobacco cigarettes due to prolonged exposure to marijuana smoke from holding it in the lungs, commented Timothy A. Collins, MD, of Duke University's Pain and Palliative Care Clinic.

"As a physician, I don't think I would ever encourage a patient to smoke anything," he said in an e-mail.

Some states, such as California and New Jersey, have made marijuana "legal" for medical use, but this conflicts with federal law, and the U.S. Drug Enforcement Agency doesn't recognize its legality in those states.

"In states where there is no 'medical marijuana law' patients buy marijuana from criminals (drug dealers)," Collins added. "For pain patients this becomes a problem, as they are already on desirable prescription pain medications and subject to pressure to trade prescription narcotics for marijuana."

For the new study, Ware's group randomized 23 adults with chronic post-traumatic or postsurgical neuropathic pain to receive cannabis at a potency of 0%, 2.5%, 6% or 9.4% THC over four 14-day periods in a crossover trial.

The patients inhaled a single 25-mg dose through a pipe three times a day for the first five days in each cycle, then had a nine-day washout period before going on to the next randomized potency.

The intermediate doses yielded pain relief between the 0% THC placebo and the full-strength 9.4% dose. However, none were significantly effective compared with placebo.

Among the secondary effects, patients reported falling asleep more easily (P=0.001) and more quickly (P<0.001) with a greater feeling of drowsiness (P=0.003) and less wakefulness (P=0.01) while on 9.4% THC compared with placebo.

Mood and quality of life overall, though were unaffected.

Marijuana smoking appeared well tolerated, although the researchers noted that this may have been affected by the fact that most patients reported prior experience -- although not recent or extensive -- with the drug as an early ethics requirement to enter the study.

Patients rarely got high on the single hit they took through a pipe three times a day as part of the study, Ware's group noted.

None of the analgesic doses got plasma levels even halfway to the typical level seen among recreational users, they explained.

The most common drug-related adverse events associated with the most potent dose included headache, dry eyes, burning sensation in areas of neuropathic pain, dizziness, numbness, and cough.

Collins cautioned that marijuana bought on the street, not through medical-use prescription, varies widely in potency, making the study results of unclear generalizability to the average user.

Background: Chronic neuropathic pain affects 1%-2% of the adult population and is often refractory to standard pharmacologic treatment. Patients with chronic pain have reported using smoked cannabis to relieve pain, improve sleep and improve mood.

Methods: Adults with post-traumatic or postsurgical neuropathic pain were randomly assigned to receive cannabis at four potencies (0%, 2.5%, 6% and 9.4% tetrahydrocannabinol) over four 14-day periods in a crossover trial. Participants inhaled a single 25-mg dose through a pipe three times daily for the first five days in each cycle, followed by a nine-day washout period. Daily average pain intensity was measured using an 11-point numeric rating scale. We recorded effects on mood, sleep and quality of life, as well as adverse events.

Results: We recruited 23 participants (mean age 45.4 [standard deviation 12.3] years, 12 women [52%]), of whom 21 completed the trial. The average daily pain intensity, measured on the 11-point numeric rating scale, was lower on the prespecified primary contrast of 9.4% v. 0% tetrahydrocannabinol (5.4 v. 6.1, respectively; difference = 0.7, 95% confidence interval [CI] 0.02-1.4). Preparations with intermediate potency yielded intermediate but nonsignificant degrees of relief. Participants receiving 9.4% tetrahydrocannabinol reported improved ability to fall asleep (easier, p = 0.001; faster, p < 0.001; more drowsy, p = 0.003) and improved quality of sleep (less wakefulness, p = 0.01) relative to 0% tetrahydrocannabinol. We found no differences in mood or quality of life. The most common drug-related adverse events during the period when participants received 9.4% tetrahydrocannabinol were headache, dry eyes, burning sensation in areas of neuropathic pain, dizziness, numbness and cough.

Conclusion:: A single inhalation of 25 mg of 9.4% tetrahydrocannabinol herbal cannabis three times daily for five days reduced the intensity of pain, improved sleep and was well tolerated. Marijuana eased everything for me doc. But guess what I've been 3 weeks clean today after smoking for about 20years . God I miss mAry dearly but I have an amazing opportunity to double even triple my yearly income and I've decided to jump on it. What's a break you know. Figure I could get a glance at life thru open eyes instead of chino eyes . Let me tell you this first few days was torture I actually spent two days smoking resen to help cope and plus I was out of grass. My wife made sure of that . But i couldn't eat couldn't sleep I was pretty much bed ridden for 2 days . Really felt like the end I f the world . After 20 years of heavy smoking I've become accustomed to smoking before i did anything , literally . Honestly I kind of got to a point where I was tired of having a crutch . Always wondered how and if i could enjoy life without being high never thought I could do it but hey look at me now . Appetite is back even better . Sleep is better and don't get me started on the dreams I've been having . Geesh talk about full blown movies. So vivid and so detailed . I guess since I don't smoke myself to sleep I can actually remember my dreams lol. But ya im super proud of myself and for anyone doubting your self about bettering your future don't be scared anything is possible . don't get me wrong if i could smoke I would but I can't and i needed to do this so it's all for a good cause . All you smokers out there make sure to burn a fat one for your boy !!!
 

Cannabinoids do appear to show some therapeutic promise for a number of conditions. Having subjects smoke weed doesn't seem to me to be particularly responsible. Considering these substances are fat soluble you could achieve more reliable test results through some form of purified oral administration with standardized dosages, but my guess is the researchers here are simply trying to continue to build the volume of case studies, which in itself can be an important factor. That, and they probably just like to smoke weed...
 
IMHO, pot should have been legalized years ago in all states. The US has lost hundreds of millions of dollars in revenue.

I smoked pot in my 20's and 30's at a modest level and now smoke pot as I turn 65 soon.

The lost revenue pales in comparison to the cost of the war on drugs with regard to pot.

I read an interesting statistic the other day. Far more people die each year from opioid addictions and abuse than all illegal drugs combined.

pot's a weird drug .. i have the exact opposite reaction for me it makes me very anxious and agitated. many people i know are like you however it really mellows and relaxes them. go figure.

I could of saved them the time and money to do that study in 3 words.
Yep, It Works
My wife uses medical mary for her MS and it is a God send.
It took me years to get her to try it, she is one of them good girls.
Finally her best friend suggested it, then brought some for her to try.
The rest is history.
It does not completely replace the narcs but gives her body some relief and allows her to be comfortable enough to sleep.

pot's a weird drug .. i have the exact opposite reaction for me it makes me very anxious and agitated. many people i know are like you however it really mellows and relaxes them. go figure.

I suppose everyone want to know if I toke my wives pot, I used to be a major pot head in the day so I thought, no big deal, I'll take a few puffs and just what Tendency said happened to me.
It was like I was on crack, peeking out the window blinds and shit.
My wife just about peed herself laughing at me.

But having sex on it is awesome and then hitting the kitchen for munchies priceless
 
I would never suggest using pot (or booze) for anyone! I lived quite nicely for 30 years without either. I drink now (double shot of crown royal every night) and smoke a small amount of pot almost every Friday and Saturday. BTW since I went back to drinking alcohol in modest amounts my HDL went to the mid 50's!!! I have to say that I have also increased my cardio intensity along with the regular "manly" drink in the evening so it is hard to tell the exact cause of my significant increase in HDL (for whatever health value that may or may not have?)

With all due respect, I never compared booze to pot or vice versa.

To me they are both crutches that undermine any serious stamina/strength conditioning program.

I do both now in moderation so that makes me and "ole guy" (65 next week) that likes to escape mentally! Quite frankly I have trained so hard for 40+ years I am a bit burned out. Never drinking or getting high and watching dietary intake is boring as hell. :) I will still train, cardio and lifting but I am not nearly as strict as I was for a long time.

I think booze in moderation is ok for most people; it might even be healthy. I think the same is true for pot in a much different way!!
 
No i hear you brother ! No offense taken . Me personally wouldn't recommend alcohol one bit to no one . Now pot is a different story. I've been smoking since I was 11 years old so it's almost about 20 years . Here recently quit for a new job . If you would ask me about a joint man I'd roll it for you , put it in your mouth , and light that bad boy for ya. I'm a pot activist at best and truly believe and promote all its positive aspects . Now yes smoking anything Is bad for you one way or another but then there's the good ol vaporizer which eliminates harmful toxins from smoke inhalation. Wasnt at all saying you compared both as equal but I was just stating how the both are completely opposite friend . I hate alcohol personally every time I take one sip it totally depresses me and kills my mood . After all it is a depressant . My family is plagued with alcoholics and druggies and was lucky to end up on the good side . So I've quit drinking completely got about a good 2 years from getting shit faced .
 
MJ is a miracle drug for some, for me vaping it before i sleep gives me the best night rest i have had in a long time. I have even consider a tren cycle if i could use mj to help me sleep while on this.

If anyone has any experience with it in this way please chime in.
 
Yup buds helps to sleep while on tren but only to an extent . Depends on how long you been using and how adapted your sleeping habits are. I've been smoking a long time and if i get bomb tren it don't make a difference trensomnia is inevitable
 
I just wanted to chime in regaling back to early 20's and how I was a "non-functional stoner". Get high and so paranoid can barely take the sound of a mouse creeping by and just go catatonic, ears up, and wondering what the fuck it will reveal to be... LOL.

But it started with those damned forceable trips to the hamburger joint. You know, the first trips out when you actually summonz the strength to actually ORDER, and NOW DRIVE OFF HOWLING MAD AFTER JUST INCESSANTLY LAUGHING AT THE ATTENDANT THRU THE SPEAKER... So now you realize that they dont know and cant tell. This eases that FEAR once and for all. Forevermore you are now a "Functional Stoner". I also recall finally actually heading out to the scene. And how folks saw my coming in that night and they were like "Damn you look good today!!!!". And I was like thanks I'm feeling good too.. And WHY- IT WAS THAT BIG OL HAPPY ASS SHIT EATING GRIN. Emotions are a contagion for sure..!

I am gonna lastly chime in on the subject of the POOR MAN'S BURDEN - Which is the LIE of the WATER BONG. This happens to be the one place in life where it turns out that WATER SUX..!! It is a FARCE, LIE, and CONSPIRACY OUT AND OUT to get you to WASTE YER MONEY and HAVE TO BY MORE...!!! I estimate that as much as 50% if not more of all the ingredients in the smoke get trapped in the water and thus FLUSHED DOWN THE TERLET OF ETERNITY$$$. Now I really first noticed that with a friend that always liked to use fresh water. So it may very well be that nasty old skunked up water if left in there can not uptake the smoke at all, and thus does just provide a cooling effect. But then I am thinking if i ever tried a bong again, I would just put small chunk ice in there. And DAMN I do recall you DONT want to spill that shit on yer arse...!:eek::oops:

Shit i like the water Bong but a gravity bong was way more killer and destructive . i Remember once mee and my childhood friend gilbert used an old bathtub in his garage . We filled that bitch with water to the rim .lol took about 2 hour . Then we got the big triple gallon jugs that you use on the water dispensers in offices and cut the bottom off . We used regular foil to build the bowl on the top hole and trust me that bastard was nice and chunky full of buds ! I tell you that mother was dark brown full of smoker after you pulled it up from the water and shit if that hit didn't kill us man . That jug filled up was actually like 10 hits no way in hell you could suck that much smoke!!! I will never forget that blaze we were plastered ass flat on the grass for about 2 hours !!! Ohhh good times
