Good to know.150mg / kg is the top end of the cumulative dosing prescribed by conservative dermatologists in the US, who are only concerned with facial acne (psychological impact on younger people). Provided you take it with food every time for proper absorption, make sure to continue for two months after FULL clearance, 150mg/kg gives you a very high likelihood of a permanent cure, especially since it's moderate.
However, you brought up large pores. Do you have oily skin / hair or just big pores? Large pores usually indicate large sebaceous glands, lots of oil.

Same here dude, i had bad acne on the back even natty because i have really oily skin (my head seems like i sweated sometimes). Luckily i get it only on my back (face, chest,all clear) but i got tired of this shit and also gonna try roaccutan.Good to know.
Yes, I have oily skin. No hair since I’m bald and shave it, but my head gets oily and I dislike it. I have been on topical Tretinoin .05% every night before bed for the past year and that seems to have helped shrink pore size somewhat and improved skin quality, but I am still oily and would like them shrunk more. I’ve seen you mention it can permanently shrink the sebaceous glands so wondering what your cumulative dosing suggestion for that is.
Also, say I were to start isotretinoin, do I stop using the topical Tretinoin? Or can I continue to use that for added effect?
90% of the isotretinoin I've been using on this cycle has been Accufine from Healing Pharma. There are a number of Jano tests out there of various strengths and they always show full potency. They are, of course, the proper oil filled gelcaps not UGl solid dry tabs.
They're available as 5,10,20,30, and 40mg.
The larger doses are less expensive(per mg) however. you want the flexibility to adjust your dose in small increments, especially at the beginning, so going all 40mg isn't a good idea. You also want to split into day/night dosing which helps minimize sides.
So if 220mg/kg was 20,000mg for example, I would go with:
20 strips(10 caps per strip) 40mg
20 strips 30mg
20 strips 20mg
20 strips 10mg
That way you can ease into it with 2x10mg a day for a week, then 2x20mg, then 2x30, 2x40, 2x40+10, 4x30 (ie 2x 30mg twice a day). until you get to 2g/kg max a day, if you can. These high doses are really what's required to knock out shoulder acne,, the hardest to rid.
Sides tend to worsen briefly when increasing dose then settle down so this incremental strategy makes it much easier to deal with than the big jumps usually involved with Dermatologist monthly visits.

By comparison, without insurance, using goodRX, the same regimen with generics would be around $2,000+probobly another $1k in derm visits.
PS: If cost is no concern, you could get Isotret NF, which absorbs more easily and not reliant on food, so the doses are reduced by 20% for the same efficacy. IE, 8mg=10mg. I make a point of taking the caps right after breakfast and after dinner, so it's been working. well for me with the regular types. Isotret NF is about double the cost.
I went by your titration schedule above and ordered exactly as written from Cosmic. Not sure I'm interested in the impending sides. However, I am interested in potential reduction in pore size and tightening of skin. Collagen also seems quite deficient. Over the past 10+ years, I have noticed a marked change in my skin that seems well beyond just normal aging. Used to not be this oily, and the pores on my face are huge, now. The changes did seem to coincide with starting TRT. I'll report back if I don't get depressed and eat a slug.
Not the advanced studies that show demonstrable collagen improvement in middle aged women with photo-damaged(ie, sun damaged) skin, but a literal "feels" test of pinching my 50yo flesh has shown an immediate snap back, rather than the slower return to shape, I haven't seen in a long time.
Mind you, this wasn't noticeable until I was on the other side of the mountain, around 3.5 months and 12,000 or so mg cumulative, crazy skin flaking that made me the primary source of dust in my house, and an entire manufacturing line at the lip balm factory dedicated to my needs.
It motivated me so much to go "all the way" to 220mg/kg it had me scrambling for more accufine after multiple FDA seizures with letters warning of the hazards of the letters "Rx" being in too small a font on the box to possibly be effective or safe for use by an American, Only the UGL I ordered as insurance actually arrived from that batch, and I was forced to use it for a few days until some Accufine managed to show up on a reship,

i was going to try about this much, thanks for the reassuranceIf you run accutane you dont need as much as people say, I did only like 20mg a day for a few weeks and it completly cleared up my back so just start really low.

I've already been thinking about the lip balm. I woke up in middle of night, and my lips were very dry. As far as I can tell, that's the only side effect so far... but I've been sick this past week so hard to tell what's coming from what.
Well damn. I'm only four days in, so this was the first time. I've used a similar method as yours in the past for tablets in foil packs before. I'll go back to it after this week.I did that initially but stopped after seeing visible changes in the gelcaps.
What I did was set two shot glasses next to my organizer, and used scissors to cut the strips, keeping the gelcaps sealed, and put the AM and PM doses in the shot glasses. After the AM dose I'd reload the shot glasses.
